RINGKASAN ASUHAN KEBIDANAN

Data hasil Survei Demografi Kesehatan Indonesia tahun 2012, AKI di Indonesia mencapai 359/100.000 kelahiran hidup dan AKB mencapai 32/1000 kelahiran hidup. AKI di Sumatera Utara, pada tahun 2012 yaitu 106 per 100.000 kelahiran hidup dan AKB 76 per 1000 KH. Penyebab utama kematian ibu adalah perdarahan, hipertensi, infeksi, dan penyebab tidak langsung dan kondisi medis yang sudah ada sebelumnya. Perencanaan asuhan yang dilakukan untuk mendeteksi dini agar menekan angka kematian ibu dan bayi salah satunya adalah dengan pendekatan Continuity Care.
Sasaran dilakukan pada Ny. N G3P1A1 usia 33 tahun kehamilan trimester III sampai dengan pelayanan keluarga berencana sejak bulan Februari sampai dengan April 2016 di Klinik Hj. Darmawati Kec. Medan.
Hasil yang didapatkan dari asuhan pada kehamilan sampai dengan pelayanan keluarga berencana yaitu pada pemeriksaan ANC ditemukan permasalahan yaitu anemia ringan, ketidaknyamanan seperti susah BAB, sakit pada pinggang, dan nyeri pada perut bagian bawah yang sudah dapat diatasi. Pada persalinan normal ditemukan kesenjangan dengan teori yaitu asuhan persalinan normal namun tidak terdapat masalah, BBL dan nifas pada setiap kunjungannya tidak terdapat penyulit dan komplikasi, hasil pemeriksaan dalam batas normal, dan pada pelayanan keluarga berencana telah diberikan konseling, ibu memutuskan untuk memilih KB suntik 3 bulan.
Kesimpulanan asuhan kebidanan pada masa hamil sampai dengan penggunaan alat kontrasepsi pasca melahirkan Ny. N berlangsung dengan baik dan normal dan tidak ditemukan adanya komplikasi, asuhan Continuity Care yang diberikan sudah sesuai dengan standart. Untuk mendukung pelayanan Continuity Care diharapkan kepada petugas kesehatan khususnya bidan dan klinik bersalin untuk melakukan asuhan kebidanan yang secara Continuity Care dilapangan pada saat melayani masyarakat.

The orphanage planning is carried out to detect early in order to suppress the maternal mortality and infant one is with the approach of Continuity of Care.The target is done in Ny. N G3P1A1 33 years of age III trimester of pregnancy up to family planning services since February to April 2016 at the clinic Hj. Darmawati Kec. Medan.The results obtained from the orphanage in pregnancy up to family planning services during the examination of the ANC found problems i.e. anemia of mild discomfort, such as hard chapters, pain in the waist, and pain in the lower abdomen that can already be addressed. At vaginal birth found the gap with the theory i.e. orphanage vaginal birth but there is no problem, BBL and parturition on each of his visit there were no penyulit and complications, the results of the examinations in the normal range, and on family planning services have been given counselling, the mother decided to select KB syringe 3 months.Kesimpulanan midwifery care during the pregnant up to the use of contraceptives post-birth Ny. N take place with good and normal and not found the presence of complications, care Continuity Care given is in compliance with the standard. SUMMARY CARE MIDWIFERY

Data from Indonesia Demographic Health Survey 2012, maternal mortality rate in Indonesia reached 359 / 100,000 live births and neonatal achieve 32/1000 live births. AKI in North Sumatra, in 2012 that is 106 per 100,000 live births and 76 per 1000 KH IMR. The main causes of maternal death are hemorrhage, hypertension, infections, and an indirect cause or medical condition that already exists. Care planning is done for early detection in order to reduce the number of maternal and infant mortality one is to approach Continuity Care.
Goal performed at Ny. N G3P1A1 age of 33 years trimester III to family planning services from February until April 2016 at the Clinic Hj. Darmawati district. Field.
The result of care in pregnancy up to family planning services, namely the ANC inspection found problems that mild anemia, inconveniences such as hard bowel movement, pain in the waist, and pain in the lower abdomen that can be solved. In normal deliveries found the gap with the theory that normal delivery care, but there are no problems, BBL and puerperal on every visit there are no complications and complications, examination results within normal limits, and the family planning services have been given counseling, the mother decided to choose injectables 3 months.
Conclusion the midwifery care during pregnancy through use of contraceptives postpartum Ny. N is going well and normal and there were no complications, Care Continuity of care provided is in accordance with the standard. To support the Continuity Care services are expected to health workers, especially midwives and the maternity clinic to perform midwifery care Care Continuity in the field at the time of serving the community.
