Di istana (sebutlah universitas milik Ibnu Sina) terdapat kumpulan perpustakaan yang juga menghimpun warisan kekayaan ilmu kedokteran kuna milik Hipocrates dan Gallen. Fakultas kedokteran tersebut diisi beragam pelajar dari manca negara, ras maupun agama. Bersama ruang kuliah terintegrasi sebuah ruang perawatan yang memiliki bangsal-bangsal tempat tidur sendiri sebagaimana model rumah sakit modern. Di atasnya terdapat gudang penyimpanan bahan obat dan alat medis, hingga pendingin untuk membuat es.

Bersama Ibnu Sina, Rob belajar kedokteran secara ilmiah bagaimana mulai melakukan anamnesis,pemeriksaan medis, mendiagnosa nadi, prosedur bedah minor, hingga bidang-bidang lainnya seperti filsafat aristoteles dan astronomi.


Digambarkan keindahan etika ilmu kedokteran di era itu seperti yang diajarkan Ibnu Sina pada para mahasiswanya: “Untuk mengobati suatu penyakit, perlakukan dengan baik orang yang menderita”. Dan terhadap setiap pasien Ibnu Sina mengawali visite-nya dengan kalimat sapaan “Salam, saya Ibnu Sina, dengan izinmu saya akan merawatmu ”..

Ibnu Sina bukan saja seorang Maha Tabib ketika itu, tapi juga merupakan seorang guru yang senantiasa berfikiran optimis positif. Suatu ketika di atas puncak istana dibawah teleskop, Rob bertanya pada Ibnu Sina. “Frustasikah kita, bila ternyata kita tahu bahwa kita ternyata lebih banyak tidak tahu daripada mengetahui sesuatu? Ibnu Sina menjawab: “Bukankah dunia hanya menjadi biasa-biasa saja dan membosankan tanpa kehadiran misteri?”


Kisah film ini dibumbui perseteruan antara suku bani seljuk yang hidup nomaden dan liar dipadang gurun yang bersekutu dengan para mullah “yang kaku” memahami agama, untuk memerangi Kekhalifahan Shah yang dianggap terlalu liberal. Hingga pemimpin Bani Seljuk tersebut mengirim “bom biologis” berupa penderita Black Death sebagai balasan atas kematian puteranya yang dipenggal oleh Shah. Black Death menyebabkan kegemparan dan wabah mematikan bagi kota Ishafan. Ibnu Sina meminta evakuasi seluruh penduduk, namun Shah menolaknya.


Disini sekali lagi ditampilkan jiwa kepemimpinan, sense-nya sebagai seorang guru besar dan tabib hebat, Ibnu Sina memutuskan bersama mahasiswanya bereksperimen mencari pengobatan terhadap wabah yang terjadi di masyarakat meski resiko yang dihadapi mereka adalah kematian sekalipun. Mereka disibukkan merawat banyaknya pasien nyaris tak kunjung henti. Ketika itu digambarkan, banyak warga yang mengisolir diri dengan menembok pintu rumahnya atau pergi meninggalkan kota Ishafan. Adegan akhir melawan wabah tersebut, akhirnya kerja keras Ibnu Sina menemukan jawaban, bahwa penularan wabah dapat diputus dengan membasmi vektor penyakit yakni tikus; wabahpun teratasi.


Diantara korban Black Death ada seorang bernama Qosim, penganut Zoroaster yang kebetulan dirawat Rob. Filosofi agama Zoroaster berbeda dengan agama Samawi (Yahudi, Nasrani & Islam) soal kebangkitan. Qosim berwasiat pada Rob agar bila ia mati, ia meminta tubuhnya diletakkan di atas menara agar bisa dimakan burung pemakan bangkai. Dengan demikian jiwanya terbebas. Kesempatan emas itu tak disia-siakan Rob, segera ia membawa jenazah Qosim dan membelahnya di gudang penyimpanan obat untuk mempelajari organ dalam yang ada di dada & rongga perut. Rob melukis organ-organ itu sebagaimana gambar ilustrasi di atlas kedokteran sekarang ini.

Saat Rob ditangkap para Mullah karena melakukan pencurian mayat buat obyek penelitian, Ibnu Sina juga terseret ke dalam penjara. Di dlm bui, guru dan murid ini berdiskusi ttg apa yang sebenarnya ada didalam rongga tubuh manusia. "Ceritakan Rob, jangan lewatkan barang satu hal pun" tanya Ibnu Sina. "Guru, tubuh manusia menyimpan misteri keindahan sekaligus hal yang menakutkan" kata Rob.

Pengetahuan itu yang kemudian mengantarkannya dapat melakukan operasi appendektomy (pengangkatan usus buntu) yang mencekam pada khalifah Shah. Dalam adegan itu, ada visualisasi yang mengharukan dimana Rob berfantasi, berhasil melakukan operasi pengangkatan usus buntu kepada Ibunya dalam keadaan ibunya tersenyum padanya.
