Pengkajian pada Tn. P tanggal 6 Jul

Pengkajian pada Tn. P tanggal 6 Juli 2014 di Ruang Kenanga RSUD Kebumen didapatkan data klien mengatakan nyeri post operasi pada kaki sebelah kiri ( cruris sinistra ), nyeri seperti ditusuk tusuk, skala 6, nyeri terus menerus, tekanan darah 119/46 mmHg, nadi 110 kali/menit, respirasi rate 23 kali/menit, suhu 36,4oC.
Nyeri akut berhubungan dengan agen cedera fisik ( post operasi ), intervensi yang telah dibuat yaitu pengkajian nyeri secara komprehensif ( PQRST ), observasi nonverbal dari ketidaknyamanan, ajarkan teknik non farmakologi ( distraksi relaksasi dan nafas dalam ), evaluasi nyeri, tingkatkan istirahat, kolaborasi dan berikan analgesia untuk mengurangi nyeri, monitor vital sign, dan implementasi sudah dilakukan semua sesuai intervensi. Evaluasi yang dilakukan selama 3 hari, masalah keperawatan nyeri akut berhubungan dengan agen cedera fisik ( post operasi ) teratasi.
Rekomendasi dari berbagai penelitian tentang pemenuhan kebutuhan rasa aman dan nyaman khususnya nyeri akut didapatkan hasil efek musik lebih efektif karena mampu menurunkan intensitas nyeri pada post operasi yang diukur dengan Numerical Rating Scale ( NRS ), dan dapat memberikan rasa senang, ketenangan dan kenyamanan secara psikologis.
